Knee Pain and ACL Rehab FAQs

Do I need an MRI first?
Usually not. Imaging changes the plan when there is real trauma, when the knee locks or gives way, or when it is not responding. Otherwise it tends to find wear that was already there and was never the thing causing your pain.

Do I need surgery for a meniscus tear?
Often not. Degenerative tears in adults do about as well with loaded rehab as with a scope. Locked knees, large unstable tears, and young athletes with true mechanical symptoms are where that conversation actually belongs.

How long is ACL rehab?
Most athletes are looking at nine to twelve months before return to sport, and that is a criteria decision, not a calendar one. Strength, hop symmetry, and how the knee handles cutting decide it, not the date on the operative report.

Can I keep training with knee pain?
Usually yes, with changes. We adjust load, range, or volume and keep the rest of the body working. Shutting an athlete down completely is rarely the right call.

Is it my knee or something above it?
Not always obvious from where it hurts. Weak hips, a stiff ankle, and poor trunk control all show up as knee pain, which is why we assess the whole chain instead of only the sore joint.

I already did physical therapy and it did not work. Now what?
That usually means the loading stopped too early or never got heavy enough. Most knees that failed rehab were never actually loaded to the demands of the sport they were going back to.

Do I need a referral to be seen?
